The Role of Primary Care Partnerships

The Primary Care Partnership Strategy

The PCP Strategy is funded by the Victorian government to improve the health and wellbeing of community members by strengthening relationships between primary care providers across a catchment area so they are able to implement improved service coordination, integrated health promotion and planning and joint programs.

Aims of the strategy include to:
  • Improve the experience and outcomes for people who use primary care services.
  • Reduce the preventable use of hospital, medical and residential services through a greater emphasis on health promotion programs, early intervention and chronic disease management.
  • Develop service coordination links between primary care providers to enable early needs identification and service planning, and effective and efficient referral and care coordination between GP’s and other healthcare agencies.
  • Plan and deliver more effective health promotion programs and services, underpinned by a social model of health.
  • Increase community participation in needs identification, and service development and implementation.

Strategic Objectives

CWGPCP Strategic Priorities and Goals 2009- 2012

The CWGPCP Partnership Plan 2009-2012 focuses on three strategic priorities, identified through an analysis of relevant social and health data and extensive consultation with member agencies.   Priorities, goals and desired longer term outcomes are:

1.      Building community resilience

  • Increase social connectivity and healthy life choice opportunities in small towns and vulnerable communities - Stronger, connected communities offering a range of healthy activities.
  • Increase the capacity of schools to support wellbeing and build resilience in young people - Supportive, health promoting schools and stronger young people).

·         Create communities that embrace diversity and are safe and inclusive for all -Increased participation in community life by all.

2.      Reducing health inequalities

·         Improve the health and wellbeing of refugees in Latrobe City - Improved health, wellbeing and participation by refugees.

·         Enable healthy choices and opportunities for people on low incomes -  More people participating in healthy activities.

·         Create healthy and supportive environments for children - Healthy choices are known and easily accessed.

3.     Achieving better health outcomes for people with chronic or complex conditions

  • Enable streamlined and efficient access to needed services - Primary care services that are welcoming and easy to access.
  • Strengthen collaboration and coordination between general practice and other service providers in Gippsland - All types of services and practitioners working together to provide better care.
  • Support organisational and systems change to improve the management of people with chronic or complex care needs - People able to manage health conditions with the coordinated support of agencies.

CWGPCP Structure

To enable organisations across the catchment to participate in relevant Central West Gippsland Primary Care Partnership activities in line with their capacity at that time, a number of working groups have been established around key strategic areas or joint initiatives.  Organisations may choose for a range of workers to participate at any or all PCP meetings, though have only one voting right at business meetings.
